Reason For Aseptic Technique Is To Maintain Asepsis Nursing Essay Reflection 2 Aseptic Non-Touch Technique This reflective piece will be about a procedure that took place whilst on placement and will look at infection control whilst doing an aseptic non-touch technique (ANTT). Using Atkins and Murphys 1993 model of reflection (Rolfe, G et al 2001). I have used this model as it helped me become aware of my actual thoughts and feelings regarding the situation, and more focused when analyzing it. Description I visited a lady who had bilateral leg wounds that needed to be dressed daily, the leg dressings that had been put on the day before were heavily exudated, the lady who I am going to call Jane to protect her identity as per NMC 2004 was adamant that she had kept her legs raised. The nurse that I was working with said that she would dress the legs this time and when I visited again I would do the dressings. The nurse opened the sterile packaging of gloves and placed it on the floor, this was to be her sterile field. The dressings and bandages that she required were opened and placed onto the sterile field and the nurse placed an apron over her uniform. The sterile gloves were put on and she proceeded to remove the dressings that were on Janes legs, once the dressing had been removed these were placed into the waste bag. The nurse then proceeded to assess the wound and then redress it without changing her gloves, by not changing her gloves which were originally sterile these will now have become contaminated by handling the soiled dressing and would contaminate the new dressings and the wound. Stage One Whilst watching the nurse change the dressing this made me question what I had been taught in University about ANTT. The procedure that we were taught in university is from The Royal Marsden 2008. This made me feel uncomfortable and question that the patient would be at risk from healthcare associated infections (HCAI). Stage Two The reason for aseptic technique is to maintain asepsis and helps to protect the patient from HCAI it also protects the nurse from any of the patients body fluids and toxic substances (Department of Health (DOH) 2005). Elderly people are more at risk of infection which is caused by organisms that invade the immunological defence mechanisms as there immune systems are less efficient ( Calandra 2000) ANTT is used to ensure that when a healthcare professional handles sterile equipment only the part of the equipment that will not contact the wound is handled (Preston 2005). When doing the ANTT procedure this involves ensuring that consent has been gained from the patient and they are aware of what the procedure will be, the environment and the equipment is prepared, hand-washing takes place, personal protective equipment is used and a sterile field is maintained. In a Primary Care setting one of the biggest problems is infection that enters the body through a tear in the skin, this may b e through a leg ulcer. A small number of microbes are sufficient enough for an infection to be caused, this may then be difficult to treat with antibiotics and what may have been a trivial problem may end up becoming a significant problem (DOH 2007). Nurses should assess the risk of transmission of infections from one person to another and plan their nursing care accordingly before they commence any form of action (Chalmers Straub 2006). Hand washing prior to ANTT has been found to be the most important procedure for preventing infections, hands can be the biggest route of transmission of infection if not washed correctly (Akyol et al 2006). There have been recent studies that show that hand washing is rarely carried out in a satisfactory manner and 89% of staff misses some part of their hand surface during hand washing (Mcardle et al 2006). Handwashing should be done prior any procedure, this can be achieved by three different methods:- Soap and water this is effective in removing physical dirt or any soiling and micro-organisms liquid soap is more efficient than a soap bar (Ehrenkrantz, 1992). Anti-microbial detergent which is effective in removing physical dirt and is more effective than soap in removing micro-organisms (Ehrenkrantz, 1992). Alcohol based hand rubs, these are not as effective as the above in removing dirt or soil but are more effective in destroying transient bacteria (Storr, J, Clayton-Kent, S, 2004). An aseptic procedure should be done in a clean environment and any equipment used should be sterile and disposable or decontaminated after each use and the nurse should ensure that the equipment is free from dust and any other soilage (DOH 2003). Whilst the nurse does ANTT procedure it is essential that her hands, even though they have been washed, do not contaminate the sterile field or the patient, this is achieved by the nurse using sterile gloves she needs to be aware however that gloves can be damaged during use and may no longer be sterile (Kelsall et al 2006). The aim of wearing sterile gloves is to reduce the risk of cross-infection from nursing staff to patients and to also reduce the transient contamination of the hands by micro-organisms from one patient to another (infection control Nurses Association (ICNA) 2002). The ICNA recommend that before a patient is treated a comprehensive risk assessment is taken to determine the most appropriate glove type for the task to be undertaken. Its been suggested that sterile gloves are only necessary if the nurses hands come into contact with the patients sterile body area, they argue that non-sterile gloves provide adequate infection control if hands decontamination has been done effectively (Hollinworth and Kingston 1998). Factors that need to be considered when making the choice between aseptic or clean technique for wound care is the setting where the dressing is to be done, the immune status of the patient, this is influenced by age, medication, type of wound, location and depth of wound and the invasiveness of the procedure (APIC 2001). Hartley (2005) reports that aseptic technique is not being carried out to a high standard and this could be related to the theory-practice gap or complacency in the professional field. Improving the skill based care needs to be the main focus on post -registration education this includes which gloves to choose, maintaining a sterile field with the risk of non-touch technique and also developing assessment protocols (Preston 2005). I have some little girl whose father teaches her how to read and write.I confronted her about it telling her kindly to have her father not teach her anymore, so we could start off fresh to a new year of teaching, and she lies to me! Apparently her father does not teach her anything, but I ain't buying it! No one even learns about writing until the 3rd grade that is without the help of another person. She was even giving me lip about another person! That Walter kid or whatever his name was. The least he c ould have done was accept my damn quarter! I had to whip Scout or else she was going to give me more lip. That wasn't even the worst of my troubles today!Some little kid named Burris shows up to class with lice in his damn hair! After I told him to take the day off too clean him, he stated that he's only here for the first day. Thank god. But after that, he starts yelling at me and having an attitude. I told him that if he doesn't leave the class immediately then I was going to call the principal. He started saying some really offense language to me that really hurt me. Thankly after he was done, he left. This crap better not is going on for the rest of this year. The purchase manager of Noon Pakistan Limited is Mr.Malik Ashraf who has been with the firm since its establishment. The operations of the department and interview findings are described as follows:- The purchase department is responsible for purchasing all materials required by the head office and plant other then milk which is purchased at the milk collection centre in Bhalwal. Materials which are purchased by the department range from ordinary items like office and plant stationary to complex materials like Flavors, Spare parts for plant equipment and transportation vehicles, Chemicals, Fuel and everything related to the manufacturing process, Logistics and day to day operations. Demand for requisition comes from the factory and approval has to be given by the Managing Director. The purchase department currently has 4-5 employees. The purchase department stays in coordination with the plant and suppliers through phone and email and as such no software is currently being used by the department. The department uses a manual system of keeping records and storing files. The purchase department always keeps 3 copies of requisitions and delievery challans. One copy is sent to the pl ant, one copy is maintained with the purchase department and one copy is kept by the Purchase Manager. The major suppliers of Noon Pakistan Limited are SMC, Sitara Chemicals , Jeson Foods and Danisco. Everyone is involved in the purchase process including the General Manager, Purchase Manager and people from the plant. Payments to suppliers are made either on cash or credit through the head office. Credit terms depend on contacts and relations with suppliers and can range from 15 days up to a month. (Ashraf) According to Mr. Malik, Noon Pakistan Limited believes in maintaining healthy relations with Suppliers and this is one of the key strengths of the department. The primary reason given for this was that Juices and Milk factories in Pakistan have no second option since suppliers are limited and few. They have to purchase materials from the specified suppliers and bargaining power of suppliers is high in this regard. However if relations are good with suppliers not only do suppliers cooperate but may also be lenient in terms of credit. In this regard price does not play a major issue but a greater issue is that of availability of materials from suppliers. Mr. Malik also stated that in case of materials needed from local suppliers, the purchase department is given no time and purchase has to be made immediately. However when imported materials are required from foreign suppliers, the Purchase department is informed in advance as shipping takes time. Mr. Malik also stated that there is no fixed pattern of purchases made throughout the year and it all depends on plant and market requirements. The quality of materials purchased is checked at the plant and if there are defects the merchandise is returned to the supplier. Mr. Malik also stated that there is no HR department as such and the performance of the purchase department is monitored from the plant and the Managing Director. (Ashraf) When the Purchase Department gets a requisition from the plant, the respective supplier is contacted and a van or truck of the firm goes to the supplier along with delievery Challan. The goods are then transported to the plant. One issue which is being faced by the purchase department is lack of coordination with the plant. Mr.Malik pointed out an instance in which the plant actually required Diesel but in the requisition form mentioned Furnace Oil. He mentioned some employees in the plant try to trap the purchase department to lower its performance but due to his vast experience of working with the firm and keeping a personal record of purchases, he immediately gets to know about such issues. Mr.Malik described having more informal relations with local suppliers but with foreign suppliers a formal relation is kept. This is the primary difference in dealing between local and foreign suppliers. He also mentioned that Government Policies and Regulatory Environment do have an impact on prices because if the government increases prices, suppliers also have to increase their price. Distribution is limited in Quetta and Multan because of heavy freight expenses. The process of forecasting yearly sales is that last years closing sales are doubled and the amount is set as the Sales Target for the next year. Keynes or â€Å"Keynesian economics is based on the notion that government can boost employment or cut inflation by manipulating the demand side of the economy—increasing government spending and expanding the money supply to boost employment and doing just the opposite to hold d own inflation.† â€Å"The two decades between 1919 and 1939 had seen great economic instability.† The economic troubles were being experienced both at home and abroad. The Feminine in William Butler Yeats' Poetry William Butler Yeats had a long history of involvement with women. He was deeply affected by all types of women; from love interests with Mrs. Olivia Shakespear, Maud Gonne and her adopted daughter Iseult, to a partnership and friendship with Lady Gregory, to marriage with Georgie Hyde-Lees, and finally the birth of his own daughter Anne Yeats. These relationships are reflected in his poetry on many different and multi-layered levels. The mentions of women in his work gives the readers some historical content as well as show the development of his feminine idea. As different as his many relationships with women were, so was his reflection of them in his writing. Yeats took people he knew and transformed them into images and patterns of order (Unterecker 12). In this case, it is important to notice Yeats's use of biography within his poetry. In 1889, Yeats was introduced to Maud Gonne who became a central part of his life and major theme in his work. She was an adamant Irish nationalist and a beautiful woman to Yeats. He pursued her much of his life, only to be rejected repeatedly by her. In 1894 he met Mrs. Olivia Shakespear whom he regarded as a confidant, despite their short affair. Yeats found friendship and maternal caring in Lady Gregory. "She became for him an image of aristocratic courtesy" (Untereker 14). Yeats spent time at her estate, Coole Park, and traveled often with Lady Gregory and her son Robert. He became director of Abbey Theatre with Lady Gregory in 1906. In 1911 he met George Hyde-Lees and due to the encouragement of Mrs. Shakespear and Lady Gregory he proposes. A Speech of Health Essay

Hello students, faculty, members of the school board, Superintendent, and parents. When I was first told that I was going to be the speaker for graduation, I was overcome. I was overcome with pride and excitement, as I’m sure we all are today as we close this chapter on our lives, but most of all, I was overcome with a feeling. A feeling that I have fooled you all into electing a Canadian to speak on this very important occasion. (Canadian Accent) I bet about now, you are all finally realizing just what it is you’ve done, eh? But it’s too late, eh? I can’t be stopped†¦. (Drape Canadian flag over top of podium) The rich history of Canada is a very long and interesting one. One that begins in the early years of. †¦ (Trail off and give a moment of thought)† **remove flag† or better yet have someone do it for you. ) Seriously though, after I got over my initial reaction, I realized that this was going to be a very difficult undertaking. So I did what any teen-age boy would do in a similar situation. I turn to my friends! see more:welcome speech for sports day I received two pieces of advice that not only helped me with the situation at hand, but I feel will be the predominant source of motivation for the next big step in my life and everything that follows thereafter. These two pieces of advice were. (Hold up palms with numbers inked on them) No. 1: Don’t Screw up! No. 2: Be Funny! Obviously after these two gems of advice, I was prepared to write a stunning and powerful speech that will remain with every one of us till the day we die. Or at least until one of the real speakers come up here and says something meaningful. It’s hard to believe that we’ll all be leaving this place finally. It is even harder to believe that from now on, we’re going to pretty much have to fend for ourselves. I think that whether we know it or not, we’ve gained a lot of the tools that we’ll need to make it out there, right here at Welch. Personally, I’ve learned some of the most important messages of my life within these hallowed halls. I’ve learned that it’s important to walk the road less traveled, because after a while, running down freshman in the packed hallways just loses some of its appeal. I’ve learned that no matter how good an idea may seem at the time, sometimes a giant metal dollar sundial that never sees the sun, could possibly be a bad idea! I’ve learned the importance of sticking strong to your ideals and not giving in to peer pressure, because we have to be ourselves, we have to think with our own mind! (look over at Mr.  Quinn) Is that good Quinn? Is, is that all right? Oh, ok. And no matter who’s doing it! Dammit! We will NOT CHEER! Finally, and probably most importantly, from the ever vigilant and all knowing administration, I’ve learned that a girl’s belly button is a dangerous and powerful thing that should be covered, feared, contained, kept from the world, for the good of all people everywhere. If you are able to decipher any sort of message out of what I’m saying today at all, it should be that life shouldn’t be taken so seriously. When we’re all off in college, or working at our jobs, or still living off of our parents, and we look back on our lives here at E-W, we’re not going to remember the tests we aced, or the classes we took, or the endless nights in front of a computer screen. But instead we’re going to remember the good times we had, the parties, the laughs we shared, but most of all, we’ll remember the people we knew. I was once told that you’re only as good as the people that you surround yourself with. Well, there is not a higher caliber of people that I would rather surround myself with then you all sitting in front of me today. I am proud to say that I was a part of the Welch Class of 2006. Don’t ever forget where you came from, and always remember all of the great and unique people that you shared your four years with, and when we all come back and see each other again for our 20-year reunion, there are going to be all sorts of surprises. By that time Charlie Guo will probably be ruling the world with an iron fist, Katherine Piercy will be the new reigning arm wrestling champion of the world. Jon Goodwin will have his very own morning talk show called â€Å"Wake up with a cup of Jon†, Kamiya Woodard will be the new host of â€Å"The Weakest Link† and rename it â€Å"Man! You don’t know what you talking about! † Eunice Song will be†¦ well, let’s be honest, there really isn’t much hope for Eunice, (turn to Eunice) Eunice I guess you should have just tried a little harder. The entire cheerleading squad will find a permanent role on the â€Å"Young and The Restless†, when the director is surprised to find that intense daytime drama comes surprisingly easy to them. Finally, as I’ve been saying for four years now, we’ll all be shaking the hand of our President of the United States†¦. Steve Brown.
